Injury Claims

Players and/or non-playing officials (eg. Coaches, Managers, Referees, Committee Members, Volunteers etc) who are whilst engaging in activities at SUFC sanctioned by Football Queensland will need to:


  1. Complete a Injury Report Form, which will be sent to SUFC for confirmation.
  2. If a SUFC player or official, lodge a claim through the Football Queensland insurance program if you wish to make a claim.
  3. If not a SUFC player or non-playing official, contact your club for the process they use.

The club declaration form should be emailed to us here. This forms part of the claim you lodge with FQ/Gow Gates.
